WPATH Letters & Gender-Affirming Care Assessments in Pennsylvania and New Jersey
For many transgender, nonbinary, and gender diverse individuals, documentation from a mental health professional is required to access medical transition services. Ascension Point offers gender affirming letters of support grounded in current WPATH and professional standards, while honoring your autonomy and lived experience.
Our evaluations are collaborative, respectful, and focused on understanding your goals, not challenging your identity. In most cases, a letter can be provided after a focused assessment session that explores your history, your transition needs, and any relevant mental health considerations. We aim to make the process transparent, affirming, and free from unnecessary obstacles.
These letters may support access to hormone therapy, surgical procedures, name or gender marker changes, or other gender affirming services. Above all, our approach centers dignity, authenticity, and support ensuring you feel affirmed throughout the entire process.

How many appointments are required?
For many people, the assessment can be completed in one appointment. If additional information or follow-up is needed, we’ll talk about that together. The goal is to make the process thorough without creating unnecessary barriers to your care.
Do I need a letter for gender-affirming surgery?
Some providers, surgeons, or insurance companies require support letters, which WPATH provides. Letters may support a variety of gender-affirming procedures, including chest or breast surgery, hysterectomy, facial procedures, and genital surgeries. Because requirements vary, we’ll review what your specific surgeon, provider, or insurance company is requesting before completing your documentation.
How much does a WPATH letter cost?

Can I get a WPATH letter through telehealth?
Yes. Ascension Point provides WPATH assessments through telehealth for eligible clients located in Pennsylvania and New Jersey.
What happens during WPATH letter assessments?
We’ll talk about your gender journey, the gender-affirming care you are pursuing, your goals and expectations, your understanding of the procedure, and other information relevant to your care. This is a collaborative conversation, not a test you have to “pass.”
Do I need to be in ongoing therapy to obtain a WPATH letter?
No. You do not need to establish an ongoing therapy relationship just to receive a letter. WPATH’s Standards of Care emphasize individualized assessment, and ongoing psychotherapy is not inherently required simply because someone is pursuing gender-affirming medical care.
Do I have to prove that I'm "trans enough" to qualify?
Absolutely not. Our approach is affirming and collaborative. Your identity belongs to you. The purpose of the assessment is to document the information relevant to your care and help you navigate requirements that may exist through your surgeon, medical provider, or insurance company.
Can you provide a letter for hormone therapy?
Potentially, yes. However, many providers use an informed-consent model and may not require a mental health letter for hormone therapy. If your provider specifically requires documentation, contact us so we can determine what they need.
Will my letter be accepted?
Requirements vary between surgeons, healthcare systems, and insurance plans. We encourage you to obtain your provider or insurer’s specific requirements before your appointment whenever possible so we can make sure the documentation addresses what they are requesting. SOC8 itself notes that when a letter is required, a single written opinion from a qualified healthcare professional should generally be sufficient, though individual organizations may have their own requirements
What if I'm nonbinary?
Nonbinary people can pursue gender-affirming medical care, too. Your gender does not need to fit neatly into a binary for your goals to be valid. SOC8 specifically expanded its guidance to address the healthcare needs of nonbinary people.
What if I'm nervous about the assessment?
That’s completely understandable, especially if you’ve encountered gatekeeping in healthcare before. Our goal is to make the process transparent, respectful, and collaborative. You are welcome to ask questions throughout the assessment and to understand what information will be included in your letter.
